WHEREAS, in return for instruction in firearms, use of premises, and for other good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, the Undersigned agrees to the following:
The Undersigned agrees to indemnify, hold harmless and defend the Grey Wolf Consulting, LLC, and any of its employees, directors, officers or agents (hereinafter "Grey Wolf"), from any and all fault, liabilities, costs, expenses, claims, demands or lawsuits arising out of, related to or connected with: the discharge of firearms; the course of instruction; the Undersigned 's participation in the course of instruction; the range, buildings, land and premises used for the course of instruction (hereinafter the "Premises"); the Undersigned's presence on or use of said Premises; and any and all acts or omissions of the Undersigned. And should any such claim, demand or lawsuit arise or be asserted in any way whatsoever related thereto, whether arising under the laws of the United States or of any State, or under any theory of law or equity, the Undersigned will indemnify, hold harmless and defend the “Grey Wolf”, from any and all costs, expenses or liability including, but not limited to, the cost of any settlement or judgment made or rendered against the “Grey Wolf”, whether individually, jointly, or in so lido with the Undersigned, together with all costs of court and other costs or expenses incurred in connection with any such claim, demand or lawsuit, including attorney's fees.
The Undersigned furthermore waives for himself/herself and his/her executors, administrators, assignees or heirs, any and all rights and claims for damages, losses, demands and any other actions whatsoever, which he/she may have or which may arise against the “Grey Wolf”, (including, but not limited to any and all injuries, damages or illnesses suffered by the Undersigned or the Undersigned's property), which may, in any way whatsoever, arise out of, be related to or be connected with: the course of instruction; the Premises, including any latent defect in the Premises; the Undersigned's presence on or use of said Premises; the Undersigned's property (whether or not entrusted to Grey Wolf); and, the discharge of firearms. “Grey Wolf” shall not be liable for, and the Undersigned, on behalf of himself/herself and his/her executors, administrators, assignees or heirs, hereby expressly releases the “Grey Wolf” from any and all such claims.
The Undersigned hereby expressly assumes the risk of entering the Premises and of taking part in activities on the Premises, which include, but are not limited to, instruction in the use of firearms, the discharge of firearms and the firing of live ammunition. The Undersigned furthermore hereby acknowledges and agrees that he/she has read, understands and will at all times abide by all “Grey Wolf” range rules and procedures as well as all other training protocols and regulations at all times. This instrument binds the Undersigned and his/her executors, administrators, assignees or heirs.
By my signature on this application, I state and affirm that I have no criminal convictions, am not currently under indictment or prosecution for any offense, and am not wanted for questioning or arrest by any law enforcement or government agency. I further state that I have no history of mental illness or substance abuse. I additionally state and affirm that the skills learned through this training program will be used only for lawful purposes.
I understand that my training may be terminated at any time during the course if my actions and / or behaviors are not deemed appropriate by the staff of Grey Wolf Consulting, LLC. Upon my signature, I agree to release Grey Wolf Consulting, LLC from any liability that may occur during the course of training or thereafter.
All participants within any Grey Wolf Consulting training course, program or seminar are required as a matter of policy to read, understand and agree to the terms and conditions contained herein.
1. All training courses or programs which offer a “CERTIFICATION” will require the successful completion of a “WRITTEN EXAMINATION” with a minimum passing score of NO LESS than 90%. Should this minimum not be met, the student shall be permitted to make additional attempts. Such attempts made on the day of the course shall incur an additional fee of $75.00 per additional attempt. Alternatively, the student shall have the option to schedule an appointment with the Instructor within a maximum of 7 days from the date of the course to make an additional attempt. Such retesting shall be completed by appointment and shall be at the time and location convenient to the Instructor and shall incur an additional fee of $100.00 per additional hour.

All training courses or programs which require the successful completion of a “LIVE FIRE QUALIFICATION COURSE” which is considered “TESTING” and not training, will require a minimum passing score of NO LESS than 260. Should this minimum not be met, the student shall be provided the following options below.

Grey Wolf Consulting, LLC reserves the right to immediately terminate any training engaged in by the participant at any time at its own discretion for any behavior or actions which is deemed to be unsafe, disruptive or disrespectful and such termination of services provided shall cause any tuition or payments made to Grey Wolf Consulting, LLC to be forfeited and retained by Grey Wolf Consulting, LLC.
4. For those who are enrolled within the State of Connecticut Security Officers Firearms Permit (Blue Card) Course, it is understood that although the class may be attended and completed prior to being employed by a security company, the State of Connecticut will NOT issue a Security Firearms Endorsement (Class 1) UNTIL the participant is registered and employed by a licensed security company within the State of Connecticut. It is further understood that the certification card issued within such a class or program will only remain valid for 6 months before another live fire qualification course will be required for an additional 6 months.
5. Effective 01 May, 2022 cellular telephones or any other electronic device or device capable of photography are no longer permitted within the classroom, ranges or other areas of Grey Wolf Consulting, LLC without prior authorization. Any violation of policy may result in IMMEDIATE TERMINATION from training and forfeiture of any fees or tuitions paid. Students are required to leave any such devices within their personal vehicles and such devices may only be used during breaks within such vehicles unless otherwise instructed.
6. Effective 01 October, 2022 all participants within training programs granting certification (i.e. CT. Blue Card, etc.) shall be required to have a minimum of 3 magazine / speedloaders for their duty sidearm provided such magazines maintain a minimum of 10 rnds. Should such magazines maintain less than 10 rnds, such participant shall be required to have a minimum of 4 magazine / speedloaders. Failure to adhere to this condition shall result in the participant NOT being permitted onto the firing line for any live fire activities including but not limited to the required live fire qualification course until such conditions are met and may incur additional fees including instructor and range fees.
7. Students must arrive 15 minutes before the scheduled start of their seminar. Late arrivals, defined as those not present at the exact start time, will be denied entry and will forfeit their tuition. This policy is strictly enforced to maintain the professionalism, safety, integrity and schedule of the training sessions.
1. NO handling of firearms or other weapons is permitted in any manner away from the firing line. (This includes the loading, unloading, press checks, or demonstration / display, etc. to another individual). This regulation shall not include the loading of magazines or speed loaders, provided the weapon is properly secured within a carrying case or holster.
2. NO client or student shall leave the firing line or training area for ANY reason without first advising and receiving authorization from a staff instructor of Grey Wolf Consulting, LLC

ALL clients and students of Grey Wolf Consulting, LLC or Other Authorized Agent, Representative or Guest Instructor shall be required to wear appropriate eye and hearing protection at all times, whenever they are within 25’ of the firing line. This regulation shall not apply during times of lecture / discussion by the instructor, under CEASE FIRE conditions or by special authorization by the instructor.
4. NO client or student shall go forward of the firing line at ANY time for ANY reason without the authorization from a staff instructor of Grey Wolf Consulting, LLC or Other Authorized Agent, Representative or Guest Instructor. If such a move is requested and authorized, an immediate CEASE FIRE will be called, and all clients / students will immediately secure their weapon either in a holster or carrying case or on a table (If available) step back a minimum of 4’ from the weapon and not approach nor in any way handle the weapon or other weapons until the command to COMMENCE is given by a staff instructor of Grey Wolf Consulting, LLC or Other Authorized Agent, Representative or Guest Instructor.
5. In the event of a malfunction or stoppage, which cannot be cleared after 1 attempt using accepted methods, the client or student shall immediately raise their support side hand and await assistance from a staff instructor of Grey Wolf Consulting, LLC or Other Authorized Agent, Representative or Guest Instructor During this time, the weapon shall be maintained with the muzzle pointed down range at all times.
6. NO client or student shall discharge their weapon at any object or target not specifically authorized by the staff of Grey Wolf Consulting, LLC or Other Authorized Agent, Representative or Guest Instructor.
7. ALL injuries (without regard for cause) shall be reported to the staff instructor of Grey Wolf Consulting, LLC or Other Authorized Agent, Representative or Guest Instructor immediately.
8. NO personal reloads are allowed at anytime during training programs without prior authorization provided through Grey Wolf Consulting, LLC or Other Authorized Agent, Representative or Guest Instructor.
9. ALL clients and students of Grey Wolf Consulting, LLC or Other Authorized Agent, Representative or Guest Instructor are expected to clean up the training area upon termination of a training session. The area shall be left as it appeared before our arrival. All brass shall be collected and deposited within approved containers, and all trash shall be disposed of properly.
10. ANY client or student may be removed from training at any time by any staff instructor of Grey Wolf Consulting, LLC or Other Authorized Agent, Representative or Guest Instructor for any behavior deemed to be unsafe, hostile, disruptive, disrespectful, argumentative or rude. In the event of such removal, any fees or tuitions paid shall be forfeit.
12. ANY damage caused to the equipment of Grey Wolf Consulting, LLC or Other Authorized Agent, Representative or Guest Instructor to include but not be limited the shooting of target holders or other equipment to shall incur a minimum charge of $10.00 per occurrence.

14. ALL OTHER SAFETY RULES AND REGULATIONS SHALL BE AT THE DISCREATION OF THE INSTRUCTOR/s PRESENT AND SHALL BE BASED UPON THE ACTIVITIES ENGAGED IN. ANY MODIFICATIONS TO THE SAFETY RULES AND REGULATIONS WILL BE CONVEYED PRIOR TO COMMENSEMENT OF SHOOTING.
I hereby give Grey Wolf Consulting, LLC its assigns, licensees and legal representatives the irrevocable right to use my name/photograph/image/audio recording/video recording/ and likeness (“My Image”) in all forms and manner including but not limited to publication on Internet Web Sites, broadcasts and any other publications as released to or by Grey Wolf Consulting, LLC. I understand that Grey Wolf Consulting, LLC cannot control unauthorized use of My Image by persons not associated with Grey Wolf Consulting, LLC once My Image has been published. I hereby forever waive any right to inspect or approve any publication of My Image by Grey Wolf Consulting, LLC. I have carefully reviewed and understand the above provisions and agree to be bound by them. I voluntarily and irrevocably give my consent and agree to this Release and Waiver.
